Undescended Testis — Why Treatment Matters in Adults

Cryptorchidism — undescended testis — occurs when one or both testes fail to descend into the scrotum during fetal development. While ideally corrected in early childhood (before age 2), a significant number of cases are missed, undertreated, or present de novo in adulthood due to acquired re-ascent. Adult orchiopexy is a well-established procedure performed by urological surgeons to bring the undescended testis into the scrotal sac where it can be monitored, and to reduce the associated risk of testicular cancer.

An undescended testis has an approximately 4–10× elevated risk of testicular malignancy compared to a normally descended testis. The risk is higher for intra-abdominal testes than for inguinal testes. Orchiopexy places the testis in the scrotum where regular self-examination becomes possible — even if it does not fully eliminate cancer risk, it dramatically improves surveillance. In addition, some men with late-presenting undescended testis experience discomfort, torsion risk, or psychological distress that is effectively addressed by orchiopexy.

Is orchiopexy done for adults?
Yes. Adult orchiopexy is performed for men with previously undetected or untreated cryptorchidism, or for acquired testicular re-ascent. The procedure differs technically from paediatric orchiopexy but is well-established in urological surgery.
Does adult orchiopexy improve fertility?
The evidence is mixed. Early childhood orchiopexy (before age 2) preserves fertility best. Adult orchiopexy is less likely to improve fertility if the testis has had prolonged exposure to abdominal temperature.
