The no-scalpel vasectomy (NSV) is the international gold standard for male sterilisation. Introduced in China in the 1970s and now recommended by the WHO and the American Urological Association, NSV uses a single tiny puncture rather than a surgical incision to access the vas deferens — the tubes that carry sperm from the testes to the ejaculatory duct.
During the procedure, both vas deferens are located by palpation through the scrotal skin and secured with a ringed clamp. A sharp-tipped dissecting forceps creates a 3–5mm puncture in the scrotal skin, through which each vas is delivered, isolated, and occluded using a combination of ligation (tying), cautery (heat sealing), and fascial interposition (placing the fascial sheath between the two cut ends to prevent recanalization). The entire procedure takes 15–20 minutes. No sutures are required — the puncture is too small to need stitching and heals within a few days.
The result is permanent sterility — sperm can no longer pass through the vas deferens into the ejaculate. The testes continue functioning normally; testosterone production is completely unaffected.
