The Procedure at Urogyn Busan

Adult circumcision is a straightforward urological procedure with well-established technique, predictable outcomes, and low complication rates when performed by a trained urologist. At Urogyn Busan, the sleeve resection technique is used — the gold standard for adult circumcision. This technique involves careful measurement and marking of the inner and outer preputial skin at the optimal excision level, ensuring symmetrical removal with accurate preservation of the frenular vessels and frenulum (unless the patient requests frenuloplasty simultaneously).

The procedure begins with a dorsal nerve block and ring block injection to completely numb the penis. The planned excision is marked with surgical pen. The prepuce (foreskin) is then excised at the marked level, haemostasis is secured, and the mucosal and skin edges are approximated using dissolvable sutures (3-0 or 4-0 Vicryl). A light dressing is applied and the patient walks out. Dissolvable sutures dissolve independently at 2–3 weeks — no suture removal appointment is needed, making this procedure particularly convenient for international visitors.

Medical Indications for Circumcision

• Phimosis — foreskin cannot retract over the glans, causing pain, urinary obstruction, or sexual difficulty
• Paraphimosis — retracted foreskin cannot return to normal position, requiring urgent intervention
• Recurrent balanitis — repeated inflammation or infection of the glans and inner foreskin
• Lichen sclerosus (BXO) — progressive scarring skin condition affecting the foreskin and glans
• Premalignant lesions — erythroplasia, leukoplakia, or condyloma requiring excision with circumcision

Elective (Non-Medical) Circumcision

Many men seek adult circumcision for religious observance (Islam, Judaism), cultural tradition, personal hygiene preference, or aesthetic preference. All of these are valid reasons. The same high standard of surgical care applies regardless of indication. Written informed consent in English is obtained for all patients, and the decision is entirely the patient's to make.
