Congenital penile curvature (chordee) — curvature present from birth, not caused by Peyronie's disease — can cause significant distress and interfere with sexual function, particularly ventral (downward) curvature exceeding 30 degrees. Urogyn Men's Clinic Busan provides penile plication (Nesbit procedure) for correction of congenital and acquired non-Peyronie's curvature. Board-certified urologist, English consultations.

Quick Answer

Penile curvature surgery at Urogyn Busan uses the plication (Nesbit) technique — appropriate for curvature under 60°. Same or next-day assessment. Surgery under spinal or general anaesthetic. Sexual activity resumed at 6 weeks. English OK.

About the Condition

Congenital Penile Curvature — What You Need to Know

Congenital penile curvature — also called chordee — is curvature of the erect penis present from birth or early development, caused by asymmetric development of the tunica albuginea rather than by plaque or fibrosis. It is distinct from Peyronie's disease, which is an acquired condition in adult men. Congenital curvature is most commonly ventral (downward) but can be lateral or dorsal. It is present during all erections and does not change over time unless corrected surgically.

Curvature under approximately 15–20 degrees is generally compatible with normal sexual function and may not require treatment. Curvature exceeding 30 degrees is more likely to cause difficulty with penetrative intercourse, and curvature over 45–60 degrees frequently requires surgical correction. The appropriate threshold for surgery depends on the individual's symptoms, sexual function, and psychological impact — discussed at consultation.

Plication Surgery — The Nesbit Procedure

Penile plication corrects curvature by shortening the convex (longer) side of the penis using permanent sutures or small ellipses of excised tunica. The procedure is performed under spinal or general anaesthetic. An artificial erection is created intraoperatively to assess the degree of curvature and confirm adequate correction. The main trade-off is a modest reduction in penile length on the plicated side — typically 0.5–1.5 cm depending on the severity of curvature corrected.

FAQ

Penile Curvature — FAQ

What is the difference between congenital curvature and Peyronie's disease?
Congenital curvature (chordee) is present from early in life, typically stable, and not associated with scar tissue or plaque. Peyronie's disease develops in adulthood, is caused by fibrous plaque formation within the tunica, and is often progressive during an active phase.
Does penile plication cause shortening?
Yes — plication shortens the convex side to match the concave side. Typical shortening is 0.5–1.5 cm depending on the degree of curvature corrected. This is the main trade-off and is discussed in detail at pre-operative consultation.
What degree of curvature requires surgery?
This is individualised. Curvature over 30° causing difficulty with penetrative intercourse is a common surgical threshold. Curvature under 20° can often be managed without surgery. The urologist assesses both degree and functional impact.
Can curvature be corrected without surgery?
For acquired curvature caused by Peyronie's disease in the active phase, injection therapy may partially reduce curvature. For congenital curvature, surgery is the only effective correction.
How long after surgery until sexual activity?
Sexual activity is typically restricted for 6 weeks after plication surgery to allow full wound healing and tunica recovery. The urologist reviews at 6 weeks post-operatively.
