About Peyronie's Disease

What Is Peyronie's Disease?

Peyronie's disease is a connective tissue disorder in which abnormal fibrous plaque forms within the tunica albuginea — the tough fibrous sheath surrounding the erectile tissue. The plaque is typically caused by repetitive minor trauma during sexual activity, triggering abnormal wound healing and collagen deposition. As the plaque matures over months, it creates a rigid inelastic zone that prevents the tunica from expanding normally during erection, causing the penis to bend toward the plaque — often with associated pain, shortening, and notching.

Peyronie's disease has two phases. The active phase(typically first 12–18 months) is characterised by new plaque formation, painful erections, and progressive curvature change. The stable phase begins when the plaque has calcified, pain has resolved, and the deformity is fixed. Treatment approach differs significantly between phases.

Symptoms
  • Penile curvature — dorsal (upward), ventral (downward), or lateral bending during erection
  • Palpable hard plaque under the penile skin
  • Painful erections — especially in the active phase
  • Penile shortening and narrowing ("waisting" or "hinging")
  • Erectile dysfunction — difficulty achieving or maintaining erection due to vascular impairment

Treatment Options

Peyronie's Disease Treatment at Urogyn Busan

Treatment is tailored to disease phase — active vs stable.

Active Phase

Non-surgical · Injection

Collagenase (Xiaflex) injection — dissolves collagen in the plaque. Verapamil intralesional injection — reduces fibroblast activity. PRP — growth factor injection to modulate scar tissue.

Stable Phase

Surgical · Curvature <60°

Penile plication (Nesbit procedure) — shortening the convex side to straighten the penis. Appropriate for curvature under 60° without significant shortening.

Severe Stable

Surgical · Curvature >60°

Plaque incision or excision with grafting — lengthens the concave side. Penile implant (if concurrent ED) — straightens the penis and restores erectile function simultaneously.

FAQ

Peyronie's Disease — FAQ

How common is Peyronie's disease?
Studies estimate Peyronie's disease affects 3–9% of men, though many cases go undiagnosed due to embarrassment. It can occur at any age but is most common in men over 40.
Can Peyronie's disease be cured?
There is no guaranteed cure, but effective treatments significantly reduce curvature, improve sexual function, and restore quality of life. Surgical correction in stable-phase disease achieves the best long-term results.
Does collagenase injection work?
Collagenase (Xiaflex/clostridial collagenase) is FDA-approved for Peyronie's disease and has shown 35–40% reduction in penile curvature in clinical trials. It is most effective for dorsal curvature of 30–90° in the active phase.
Will the curvature get worse?
During the active phase, curvature typically progresses over 12–18 months. In the stable phase, most cases do not worsen significantly. Early treatment intervention during the active phase is generally recommended to slow progression.
Can I have sex with Peyronie's disease?
Many men with mild-moderate curvature can continue sexual activity. Pain during erection typically resolves in the stable phase. Severe curvature may prevent penetrative intercourse — correctable with surgery.
Can foreigners get Peyronie's treatment in Busan?
Yes. Assessment via penile ultrasound, injection therapy, or surgical referral coordination are all available at Urogyn Busan with English throughout.
