TURP — The Surgical Gold Standard for Severe BPH

Transurethral resection of the prostate (TURP) is the most established surgical treatment for BPH and remains the reference standard against which all newer BPH procedures are compared. A resectoscope is inserted through the urethra into the bladder. An electrical cutting loop systematically removes the obstructing adenoma (inner prostate tissue), creating a wide open urethral channel. The procedure is performed under spinal or general anaesthesia with a minimum 1-night hospital stay.

TURP produces the highest and most immediate improvement in urinary flow rates of all BPH treatments. It is the most appropriate option for men with very large prostates (>80mL), significant bladder neck obstruction, bladder stones, or failure of minimally invasive procedures. The main trade-off is a higher risk of retrograde ejaculation (50–90%) compared to UroLift or Rezum.
